ABSTRACT

To evaluate the ability to predict visual outcome after penetrating keratoplasty [PKP] in patients with pseudophakic or aphakic corneal edema [PCE or ACE]. Medical records of 34 patients [34 eyes] who underwent PKP for PCE or ACE during 1994-2004 in Ahvaz were retrospectively analyzed for variables in the history and ocular examination before PKP and visual outcome after PKP. The predictive value of each preoperative variable including age, gender, method of intraocular lens [IOL] implantation, vitreous loss during cataract surgery, time between cataract and PKP surgery, and history of glaucoma or increased intraocular pressure [IOP] before PKP surgery on post-PKP visual outcome was assessed using logistic regression analysis. Odds ratio [OR] with 95% confidence interval [95% CI] was calculated for predictive factors. Mean follow-up was 23.6 months. Best corrected visual acuity [BCVA] of 20/200 or better was achieved in 17 patients [50%]. The strongest predictors of this outcome were time between cataract surgery and PKP [P=0.008, OR=3.50, 95% CI; 0.48-31.18], aphakia [P=0.027, OR=4.29, 95% CI; 0.36-114.8] and no history of glaucoma or increased IOP before PKP [P=0.020, OR=3.75, 95% CI; 0.71-21.41]. In patients with PCE and ACE who are candidates for PKP, time between cataract and PKP less than 20 month, no history of glaucoma or increased IOP before PKP, aphakia versus presence of IOL are associated with a better visual outcome

ABSTRACT

Results of filtration surgery, as a means of treating aphakic glaucoma secondary to presence of vitreous in anterior chamber, were improved when vitrectomy was combined with it. A thorough anterior vitrectomy was possible by using an anterior chamber maintained and consequently an 80% success rate could be achieved. Filtration was sited to either side of previous cataract section. Visual results were good, as 47% of cases could enjoy better vision, compared to 20% of cases who had worsening of their preoperative vision

ABSTRACT

Se evalúa el campo visual (perimetría cinética) en 16 pacientes afáquicos de diferentes tipos de glaucomas, comprobándose que la mejor forma de estudio de éste, es con la determinación de su corrección óptica con lente de contacto. Se muestra la forma de efectuar la corrección óptica, mediante la determinación de la distancia al vértice empleando un distómetro, y encontrando que el lente de contacto de mayor frecuencia fue de + 16,50 dioptrías. Se enfatiza la importancia de esta evaluación en pacientes glaucomatosos, dado que el campo visual en los afáquicos muestra, por su condición de tal, una importante reducción que puede llevar a una errónea interpretación, de su daño funcional por su glaucoma

ABSTRACT

The follow up of the patients was short and there may be failures later because of frequent lens loss or deposits on the lenses. In this study the failures which occurred did so within the first few weeks. It is probable that we can advise our patients that Extended Wear Contact Lenses are worn successfully by 70% of the patients with gratifying results.

ABSTRACT

A base de la existencia normal de la aberración esférica sobrecorregida en la córnea, comprobado experimentalmente por Mandell, sumado a la presencia de un LIO carente de A.E. subcorregida en un afáquico, y al libre juego de los reflejos pupilares que aseguren el cambio de abertura en la mirada a la distancia y de cerca, se elabora una teoría que permite explicar el fenómeno de la Seudoacomodación. Se analizan los factores determinantes para cumplir con la condición aplanática en un lente convergente en general, y éstos se aplican al LIO. De este análisis se infiere, que los parámetros para determinar la relación entre sus radios de curvatura anterior y posterior, son los mismos con los cuales determinamos su potencia. Esta conclusión se expresa por una fórmula, cuyo algoritmo se programa en una computadora a continuación del cálculo de la potencia del LIO

ABSTRACT

Contact Lens fitting in children is mainly for unilateral aphakia, unilateral high myopia and rarely for therapeutic or cosmetic reasons. There are many difficulties encountered in fitting. Certain precautions should be followed, particularly the choice of the proper type of lens to achieve the best possible visual acuity, and careful monitoring of vision to avoid the occurance of amblyopia. Analysis of the results of fitting and follow up is presented
